Responsible for individual service coordination for clients enrolled in Phoenix House adult Transitional Housing and Residential Services, including supporting a successful transition back into the community. In collaboration with the Program staff, the Case Manager supports the successful integration of medical care, housing and income/ employment assistance, substance abuse treatment, and other services within individual treatment plans.

B.  Recovery Specialist

Counselor Certification is required.
The Recovery Specialist work in collaboration with the Social Worker Case Manager and develop, maintains the client's treatment plan. S/He advocates recovery by serving as a personal guide/mentor for people seeking or in recovery from alcohol and other drug addiction and helping to remove barriers and obstacles to recovery. The Recovery Specialist also plans, implements and coordinates an array of comprehensive, individualized services for program participants and performs administrative work in support of the program.

C.  Part-Time Overnight Counselor
